The Hospice Team meets regularly to discuss individual patients and update their plan of care as needed.
Medical Directors:
Physicians with a specialty in pain and symptom management who collaborate with the hospice team and the patient’s primary care physician to oversee the plan of care.
Registered Nurses:
Specialists in hospice care and experts in pain management who are available around the clock to assist with any concerns that may arise.
Certified Home Health Aides:
Specially trained and available to assist patients with personal care and daily living activities such as bathing, shaving, etc.
Medical Social Workers:
Patient and family advocates who assist with utilization of community resources, insurance and facility issues, financial matters, funeral arrangements, and placement decisions.
Chaplains:
Spiritual Care Counselors who provide non-denominational faith support and bereavement support to patients, families, and staff members.
